Deep plane facelifts have gained significant popularity in the aesthetic surgery world, and for good reason. Initially popularized in Texas, this advanced technique offers unique benefits over traditional facelift methods, especially for patients seeking natural-looking, long-lasting rejuvenation. In my practice, I’ve increasingly turned to the deep plane facelift to address the limitations I’ve encountered with older techniques.
What Is a Deep Plane Facelift?
A deep plane facelift involves carefully separating the layer beneath the skin, known as the SMAS (Superficial Musculoaponeurotic System), from the underlying facial nerves and muscles while keeping the skin attached to the SMAS. This method allows for more effective repositioning of the deeper facial structures without excessive pulling on the skin’s surface.
Because this approach requires a deep understanding of facial anatomy and is more complex, it is essential that an experienced surgeon performs it. It results in a more youthful appearance with fewer skin creases.
Traditional SMAS Plication vs. Deep Plane Facelift
The more conventional approach, SMAS plication, involves lifting the skin above the SMAS and then tightening the SMAS layer by folding and suturing it. While this method is generally considered safe, it often requires more extensive skin elevation, which can:
- Increase the risk of hematoma (blood pooling beneath the skin)
- Lead to unnatural creasing, especially in patients with sun-damaged skin
- Limit how much rejuvenation can be achieved in certain areas
Over time, I’ve noticed that the results from SMAS plication can be unpredictable. While some patients maintain excellent outcomes for years, others experience sagging or an unnatural appearance as they age—even though the same technique was used.
