Platysmal banding, fat accumulation, or skin drooping create an aged neck.
- Bone remodeling
Even the underlying facial skeleton changes subtly over time, losing volume as the soft tissue overlying it sags.
These changes impact each person differently, which is why every facelift must be customized, and why a deep plane facelift is often the most restorative and natural-looking option.
The Five Self-Assessment Tests to Determine Facelift Readiness
Below are the most accurate and useful at-home tests I give prospective patients across Washington, D.C., Maryland, and Virginia. You can perform these in front of a mirror or using your phone camera.
Test #1: The Jawline Lift Test
Stand in front of a mirror and place two fingers just below your earlobe. Gently lift the skin and tissue upward and slightly backward.
Ask Yourself:
- Does your jawline suddenly look sharper?
- Do your jowls diminish?
- Does your lower face look noticeably more youthful?
If the answer is yes, your aging is primarily from structural descent, not just superficial skin laxity — a strong indication that a facelift could address your concerns.
Why it matters:
The deep plane facelift repositions these underlying tissues, offering the most natural correction of jowls and jawline sagging.
Test #2: The Midface Volume Test
Place your fingertips on the outer cheek near the lower eyelid and gently lift upward toward the temples.
Ask Yourself:
- Do your cheeks look fuller?
- Does this motion soften your nasolabial folds?
- Do you see a refreshed, “well-rested” look?
If so, your midface has experienced ligament weakening and fat pad descent — both corrected beautifully with a deep plane facelift.
This is especially common among professionals in the DMV who feel they “look tired,” even after good sleep.
Test #3: The Neck Angle Test
Turn your head slightly to the side and look at the silhouette of your jawline and neck.
Signs you may need surgical correction:
- Blunting of the jaw-neck angle
- Vertical neck platysmal muscle bands
- Excess skin or creasing
- Submental fat that doesn’t improve with weight loss
A facelift combined with a neck lift (platysmaplasty) is often the best solution.
Non-surgical treatments cannot meaningfully correct neck laxity once it becomes prominent.
Test #4: The “Photo Shock” Test

Look at:
- Side-profile photos
- Zoom screenshots
- Candid family photos
- Work headshots
