Structure vs Volume — The Two Keys to a Truly Youthful Face
Many patients I see in Washington, D.C., Maryland, and Northern Virginia ask a version of the same question:
- “Do I need fat grafting with my deep plane facelift?”
- “Is fat transfer necessary for natural-looking results?”
- “Will I look too full or too tight if I add fat?”
These are excellent questions — because modern facial rejuvenation is no longer only about lifting. The most youthful faces combine:
- Correct structure (deep tissues lifted upward)
- Correct volume (fullness in the right places)
- Correct contour (smooth transitions between facial zones)
A deep plane facelift is the gold standard for restoring structure. Fat grafting can, when needed, aid in restoring volume. But not everyone needs both.
This guide explains exactly how to determine whether fat grafting should be included with your deep plane facelift, based on your anatomy, age, goals, and aesthetic preferences.
Why Volume Loss Matters in Facial Aging
Everyone knows that the face sags as we age, but sagging is only half the story. The other half is deflation. This occurs because of loss of volume in both the bony anatomy and fat. In addition
With age, we lose volume in:
- The midface
- The upper cheeks
- The temples
- The lips
- The area around the mouth
- The jawline (bone resorption + fat loss)
This volume loss makes the face look:
- Hollow
- Flat
- Tired
- Less youthful
- Less vibrant
The deep plane facelift corrects descent, but it does not automatically restore lost fat that is more apparent in thin individuals. This is where fat grafting becomes valuable.
What Is Facial Fat Grafting?
Fat grafting uses your own purified fat, typically harvested from the abdomen or flanks, and injects it into specific facial areas needing fullness.
Fat grafting can help restore:
- Cheek volume
- Under-eye smoothness (when done conservatively)
- Chin support
- Jawline blending
- Marionette region softening
- Temple hollow correction
- Smile line support
Because it’s your own fat:
- It integrates naturally
- It ages naturally
- It moves naturally
- It can last permanently (between 20-60% of transplanted fat survives)
In addition, grafted fat contains a high percentage of stem cells that can differentiate into the type of cell the skin and layers deep to it need to maintain a more youthful appearance. In the DMV’s high Aesthetic IQ environment, this naturalness is a major advantage.
Deep Plane Facelift Alone vs Deep Plane + Fat Grafting
Here’s a breakdown of what each combination offers:
Deep Plane Facelift Alone: Best for Patients Who Have Plenty of Facial Fat
✔ Good cheek volume
✔ Mild volume loss
✔ Strong bone structure
✔ Thick soft tissue coverage
✔ Softer aging patterns
✔ A desire to look refreshed but not fuller
✔ A tendency to gain facial weight
Results you can expect:
- Cheeks lifted naturally from structural repositioning
- Jowls significantly improved
- Jawline sharply defined
- Neck contour dramatically enhanced
- Midface rejuvenation without added volume
For some patients, this alone creates the perfect outcome.
Deep Plane Facelift + Fat Grafting: Best for Patients Who Have
✔ Hollow cheeks
✔ Upper cheek deflation
✔ Prominent under-eye hollows
✔ Visible bone contours
✔ Volume loss around the mouth
✔ A history of filler overuse (looking “puffy” instead of lifted)
✔ Thin or delicate facial tissues
✔ A desire for youthful fullness in addition to lifting
Results you can expect:
- More youthful midface fullness
- Softer transitions between facial zones
- Under-eye hollow reduction
- More feminine or masculine cheek structure
- Subtle lip enhancement if desired
- Better long-term harmony
This combination is extremely common for thinner patients in their 50s–70s.
A DMV-Specific Perspective: What Patients Here Prefer
In the Washington, D.C. area, patients want:
- Elegant
- Natural
- Understated
- Non-obvious
- “Quiet luxury” rejuvenation
- Expression-preserving outcomes
The deep plane facelift aligns perfectly with this. But fat grafting must be done very conservatively for DMV patients. Why? Because fullness in the wrong areas, especially overfilled cheeks, is instantly recognizable in a city with a high aesthetic standard.
Most of my D.C.-area patients prefer:
- Subtle volume restoration
- Not “apple cheeks”
- Nothing exaggerated or Instagram-style
Fat grafting is an artistic procedure, and subtlety is key.
What Areas Benefit Most From Fat Grafting?
Here is where fat grafting adds the greatest benefit:
✔ 1. Cheeks (Anterior and Lateral Very Conservative Only)
One of the earliest signs of aging is cheek deflation. Fat grafting restores youthful curves — but must be subtle.
✔ 2. Temples
Temple hollowness gives the face a “skeletal” appearance. Filling the temples improves:
- Facial symmetry
- Brow position
- Side-profile contour
✔ 3. Under-Eyes
Deep plane facelifts improve the midface, but not the under-eye hollow itself, the tear trough area. Micro-fat grafting can softly blend this transition.
✔ 4. Nasolabial Fold Base
Fat placed deep, near the bone, can support the fold.
✔ 5. Marionette Lines
Fat softens the shadowing around the mouth.
✔ 6. Chin & Jawline
Subtle fat grafting can improve:
- Soft tissue support
- Lower face balance
- Harmony between the jawline and the cheeks
✔ 7. Lips (Micro-augmentation)
Subtle, natural, elegant.
Fat grafting is not about creating full lips, just restoring youthful shape and hydration. Since the lips are such a mobile area, grafted fat is not as certain to “take”.
What Are the Risks of Fat Grafting?
Fat grafting is safe when performed by an experienced surgeon.
Potential risks:
- Asymmetry
- Partial fat resorption
- Rare irregularities
- Over-filling (typically seen when done by inexperienced practitioners)
Proper technique minimizes all of these. I take a conservative, layered approach, choosing quality of contour over quantity of volume.
Longevity: How Long Does Fat Grafting Last?
Fat grafting is considered semi-permanent to permanent.
Timeline:
- 30–40% fat is absorbed in the first 3 months
- A porion of the remaining stays permanently
After fat integrates into its new location and establishes a blood supply, it behaves like your own tissue because it IS your own tisse
- It gains weight if you gain weight
- It slims if you lose weight
- It ages naturally
No maintenance required.
What About Fillers Instead of Fat?
Patients often ask:
- “Can I do fillers instead of fat during my facelift?”
The answer is yes — but fillers have limitations:
✔ Pros
- Immediate results
- No downtime
- Adjustable
- Good for fine-tuning
✖ Cons
- Temporary (6–18 months)
- Can cause puffiness
- Ideal in the D.C. region for subtle enhancement
- Cannot restore natural tissue texture
- More expensive long-term
- May distort facial anatomy with repeated use
For major facial rejuvenation, fat grafting provides:
- Better texture
- Better integration
- Better long-term value
Who Is NOT a Good Candidate for Fat Grafting?
You may not benefit from fat grafting if you:
- Prefer a more sculpted, angular look
- Tend to retain fluid easily
- Have very round or full cheeks already
- Want a sharply defined midface without added volume
- Want exclusively “lifted” results without fullness
During consultation, I will evaluate your bone structure and how fat grafting will fit your facial goals.
Should You Consider Adding Fat Grafting to Your Deep Plane Facelift?
A simple checklist:
✔ Add fat grafting if:
- Your cheeks look hollow
- Your under-eyes look sunken
- Your temples look concave
- You’ve lost significant facial volume
- Your skin looks “tired” despite its smoothness
- You want youthful softness
- You want long-term correction
✔ Skip fat grafting if:
- You prefer a more sculpted, sharp look
- You have naturally full cheeks
- You gain facial weight easily
- You want strictly a lifting effect
The Most Natural Rejuvenation Comes From Structure + Volume
Combining a deep plane facelift with conservative fat grafting can create the most youthful, natural, harmonious result, especially for patients with deflation or hollowness.
This combination offers:
✔ Natural soft fullness
✔ Long-lasting volume
✔ Restored contour
✔ Elegant midface shape
✔ Total facial harmony
