Is Lower Eyelid Surgery Right For You?
Restore a bright, refreshed appearance with lower eyelid surgery at Cosmetic Surgery Associates in Bethesda, MD. Also known as lower blepharoplasty, this transformative procedure is designed to reduce under-eye bags, improve puffiness, and smooth fine lines beneath the eyes. Our skilled, board-certified plastic surgeons are dedicated to achieving natural, youthful results that enhance your unique beauty in a safe and supportive environment.
What is Lower Eyelid Surgery?
Lower eyelid surgery is a specialized cosmetic procedure that rejuvenates the area beneath the eyes. Over time, the skin loses elasticity due to natural aging, sun exposure, and other factors. This causes the delicate skin on the lower eyelids to sag or wrinkle, creating the appearance of dark circles and heavy under-eye bags. Puffiness and excess fat below the eyes can also be addressed with this procedure. Lower eyelid surgery removes or repositions excess fat, tightens loose skin, and improves under-eye contours for a smoother and more youthful look. This procedure is tailored to each patient’s anatomy and aesthetic goals, ensuring balanced and beautiful results.

Good Candidates for Lower Eyelid Surgery
Ideal candidates for lower eyelid surgery are healthy adults who notice persistent bags, dark circles, or sagging under the eyes that do not improve with creams or non-surgical treatments. Candidates should have good skin elasticity and realistic expectations about what can or cannot be achieved with lower eyelid surgery. Many patients seek this surgery to correct signs of aging, achieve a brighter appearance, or complement other facial procedures. During your consultation, your surgeon will help you determine if lower eyelid surgery is best for your unique needs.
The lower eyelid lift involves a small incision hidden in the natural crease just beneath the lower lash line or inside the lower lids, where the scars can be easily concealed.
Lower Eyelid Surgery Technique
Through this incision, excess fat and tissues are removed or repositioned to eliminate puffiness. The remaining tissues and muscles are tightened and loose skin is trimmed to smooth the area, creating a seamless transition between the cheeks and the under-eye area. Our experienced board-certified plastic surgeons ensure minimal visible scarring and harmonious results.
Consultation
A consultation at Cosmetic Surgery Associates is the first step to receiving facial plastic surgery. During this visit, Dr. Richards, Dr. Jabs, or Dr. Magge will review your medical history, examine your eyelid anatomy, and listen to your concerns and goals. You will have the opportunity to ask questions, view before-and-after photos, and discuss potential outcomes so that you feel confident about each step of the procedure and the results you can expect.
Preparation
Preparation for lower eyelid surgery includes several important steps. Patients may be asked to adjust medications, stop taking blood-thinning medications or herbal supplements, and avoid tobacco use for several weeks before their procedure. We recommend that patients make arrangements for transportation on the day of surgery and help at home during the first few days of recovery. Maintaining a healthy lifestyle and following all preoperative instructions will promote an easy healing process and optimal results.
Lower Eyelid Surgery Procedure
Lower eyelid surgery can be performed under local or general anesthesia, depending on the extent of the procedure and each patient’s level of comfort. Once the patient is ready, an incision will be carefully made in the lower lid. Through this incision, your surgeon will remove sagging skin and excess fat, tighten underlying tissues, and reposition the remaining skin. Fine sutures are used to close the incision to create minimal scarring and avoid damaging the thin skin around the eyes. Eyelid surgery usually takes one to two hours, and most patients can return home the same day after a short monitoring period in our recovery room.
Lower Eyelid Surgery Recovery
Recovery after lower eyelid surgery is generally well-tolerated. Mild swelling, bruising, and soreness are expected in the first few days and can be managed with cold compresses and prescription medication. Some patients are able to manage their discomfort with over-the-counter medication alone. Temporary dry eyes and blurry vision may occur on the first day.
We recommend that patients keep their heads elevated to minimize swelling and promote a smooth recovery. Most patients return to work and light activities within one week, after most of the swelling and bruising has reduced. However, it will take several weeks for the body to fully heal, and patients must avoid strenuous exercise for at least four weeks. Post-operative appointments at Cosmetic Surgery Associates may be scheduled to monitor your healing progress.
Results from Lower Eyelid Surgery
Patients will notice significant improvements in the under-eye area that will continue to improve as bruising and swelling subside. Patients can enjoy a smoother contour without the look of heavy bags, fewer wrinkles, and a more youthful, well-rested appearance. Proper sun protection, skincare, and a healthy lifestyle will help you maintain your refreshed look for many years.

Brow Lift
A brow lift is a surgical procedure that addresses skin laxity and wrinkles on the forehead. Combining a lower eyelid lift with a brow lift can help rejuvenate the entire upper face, creating a more balanced and harmonious result.
Upper Eyelid Surgery
Upper eyelid surgery or upper blepharoplasty removes excess skin and fat from the upper eyelids, brightening the eyes and correcting drooping that may impair vision or aesthetics. Treating the upper and lower eyelids in a combined procedure creates a comprehensive lift and refreshes the entire eye area for striking improvements.
Facelift
The facelift can be performed with various techniques, but typically involves the removal of excess skin and tightening of remaining tissues in the mid and lower face to achieve a more lifted, youthful appearance. Combining a lower eyelid surgery with this procedure creates a smooth contour between the eyes and cheeks.
